1. About this site
This website gives general information about Meridian Psychiatry, our services, and how to contact us. It is not a complete source of medical advice. Nothing on the site replaces a visit with a clinician.
2. Who we are
Meridian Psychiatry is the practice operated by Jason Pierre, MSN, APRN, PMHNP-BC. We do not list a physical office here yet. If and when that changes we will update this page.
3. Appointments and telehealth
You can book in person visits or telehealth visits if they are offered. Telehealth may not be available in all states.Telehealth visits are subject to the provider being licensed in the state where you are located during the visit.You must provide accurate location information at the time of the visit so we can confirm licensure and legal requirements for care.
4. Before your visit
You must give us correct contact and insurance information.If you provide medical history or notes through forms or messages this becomes part of your record.If you send emergency or urgent messages through normal email or site forms we may not see them right away. Do not use the site for medical emergencies. If you are in danger or need immediate help call 911 or go to the nearest emergency room.
5. Fees, insurance, and payment
You are responsible for paying the fees for services you receive.We may accept insurance where noted. If we bill insurance you are responsible for any copay, coinsurance, or amount your insurer does not pay.If you do not have insurance or chose not to use it you must pay at the time of service.If a third party handles billing we will have required agreements to protect your health information.
6. Cancellation and no show
If you need to cancel or reschedule please let us know at least 24 hours before your appointment. Some appointment types may require more notice.If you do not give proper notice or you do not show for your appointment a no show or late cancellation fee may apply equal to the full session fee. We will tell you the fee policy when you book.
7. Communication and electronic consent
By contacting us by email or through the website you agree we may use those channels to send confirmations and routine messages. Email and standard text messages are not fully secure. Do not send highly sensitive medical details by normal email or SMS unless you accept the risk.
8. Medical records and privacy
Your medical records and how we use them are described in our Privacy Policy and Notice of Privacy Practices. That notice explains your HIPAA rights, how we use and share your protected health information, and how to contact our privacy officer. The privacy notice is part of these terms.
9. Patient responsibilities
Be honest about your medical and mental health history.Follow treatment plans and ask questions if you do not understand.Tell us if your contact or insurance information changes.Keep appointments or cancel in a timely way.
10. Limits of care
Not all conditions are appropriate for telehealth. We may recommend an in person visit, referral, or emergency care when needed.We will not guarantee specific results from treatment.
11. Third party links and services
Our site may link to other sites or use third party services. We are not responsible for their content or how they handle data. If a third party will have access to your protected health information we will put required agreements in place.
12. Intellectual property
Content on this site such as text, images, and logos belong to Meridian Psychiatry unless we say otherwise. You may view and print pages for your personal use only. Do not reuse or republish our content without written permission.
13. Limitation of liability
To the fullest extent allowed by law our legal responsibility to you for any claim about the website or care is limited to the money you paid us for services in the 12 months before the claim. We are not responsible for indirect damages such as lost income or emotional loss.
14. Indemnification
You agree to cover us for claims that come from your misuse of the website, your false statements, or your failure to follow these terms.
15. Disputes and governing law
We will try to resolve problems by talking first. If we cannot agree then any legal dispute will be handled in the state where the provider is licensed. If the provider holds licenses in more than one state the applicable state will be the state of licensure for the service provided. This clause does not remove your rights under consumer protection laws.
16. Changes to these terms
We may update these Terms and Conditions. When we change them we will post a new effective date. If you keep using the site after changes that means you accept the new terms.
17. Termination
We may limit or stop providing services to you for reasons such as nonpayment, repeated missed appointments, or behavior that threatens staff safety. If we end the clinician patient relationship we will help with a safe transition of care when possible.
18. Emergency and crisis
If you are in crisis or have thoughts of harming yourself or others call 911, or call your local emergency number, or go to the nearest emergency room. If you are in the United States you may also call 988 for the Suicide and Crisis Lifeline.
